10 Tip-Apex Distance in DHS & IM Nail Fixations for Hip Fractures

چکیده

Abstract Aim Dynamic hip screws (DHS) and intramedullary nail (IMN) fixations have been successful in treating extracapsular fractures. Tip to apex distance (TAD) is defined as the total sum of from tip lag screw femoral head on both AP lateral radiographs. Baumgartner et al. TAD<25 mm be predictive a result, with reduced rates metalwork failure. Therefore, this study aims assess if TAD principles are practiced during DHS IMN fixations. Method 102 patients were identified between December 2019 February 2020. The was measured radiographs calculate TAD. Patients followed up for 1 year where fractures either united or failed. Results recruited had age ranges 19–103 years old, which 36% male 64% female. 68% underwent while 32% IM nail. In group, 13 >25 mm, 28 20–25 18 15–20mm 6 <15 mm. For there 7 10 Conclusions There no nails that exceeded 25mm, but >25mm. 12/13 >25mm performed by trainees. We advocate emphasis principle re-enforcing orthopaedic trainees improve outcome neck femur

Awareness of tip-apex distance reduces failure of fixation of trochanteric fractures of the hip.

We compared the results of the surgical treatment of trochanteric hip fractures before and after surgeons had been introduced to the tip-apex distance (TAD) as a method of evaluating screw position. There were 198 fractures evaluated retrospectively and 118 after instruction. The TAD is the sum of the distance from the tip of the screw to the apex of the femoral head on anteroposterior and late...

Clinical Outcomes of Intramedullary and Extramedullary Fixation in Unstable Intertrochanteric Fractures: A Randomized Clinical Trial

Background: The best method for repairing intertrochanteric fractures is still controversial. The fixation methods includeextramedullary (EM) and intramedullary (IM). Studies that compare IM and EM fixations for unstable hip fractures arerare. In this study, our goal was to compare the efficacy of EM and IM fixation in treatment of unstable intertrochantericfractures.Met...

The prognostic value of tip-to-apex distance (TAD index) in intertrochanteric fractures fixed by dynamic hip screw

Intertrochanteric fractures (ITFs) are the most common type of fractures requiring surgical intervention. They also have the highest surgical mortality among orthopedic operations. Among the many different techniques used for fixation of this type of fracture, use of the Dynamic Hip Screw (DHS) has gained wide acceptance. This current study was designed to assess positive predictive value of ti...

Humeral Tip-apex-distance as a Prognostic Marker for Proximal Humeral Fractures in 203 Patients

BACKGROUND Humeral head fractures and their postoperative outcome remain a challenging problem in surgical daily routine. Predictive factors for loss of fixation are rare. OBJECTIVE Determination of predictive factors for the failure of osteosynthesis with the loss of fixation or migration of screws in humeral head fractures. METHOD From 1995 to 2011, 408 patients with proximal humeral frac...

Should the tip-apex distance (TAD) rule be modified for the proximal femoral nail antirotation (PFNA)? A retrospective study

BACKGROUND Unstable proximal femoral fractures are common and challenging for the orthopaedic surgeon. Often, these are treated with intramedullary nails. The most common mode of failure of any device to treat these fractures is cut-out. The Synthes proximal femoral nail antirotation (PFNA) is unique because it is the only proximal femoral intramedullary nail which employs a helical blade in li...
